DRUG TESTING CONSENT FORM<\/a><\/span>
\n*Your consent forms expire after each school year.<\/em><\/p>\n

Academically, you must be in constant communication with your
\nGuidance Counselor.\u00a0 You must have 30 credits from your previous
\nfull year, and 15 credits from your previous semester to be considered academically eligible. You also must carry 4 hours of workload in a
\ncurrent semester.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n
