The Physical Packet<\/a> must be completed once annually prior to any sports participation.<\/p>\n

The Updated Health Questionnaire<\/a> must be completed prior to participation in each sports season.<\/p>\n

The state of NJ is requiring student-athletes and parents to learn more about the dangers of opioids. Athletes and parents must read the Opioid Fact Sheet, available on Parent Portal<\/a>, and initial as acknowledgment before participation in a sport.<\/p>\n
