The Physical Packet must be completed once annually prior to any sports participation.
The Updated Health Questionnaire must be completed prior to participation in each sports season.
The state of NJ is requiring student-athletes and parents to learn more about the dangers of opioids. Athletes and parents must read the Opioid Fact Sheet, available on Parent Portal, and initial as acknowledgment before participation in a sport.
