If your child has an up-to-date physical on file, please PRINT, complete and turn into the Athletic Office the “Health History Update Questionnaire.” If your child does not have an up-to-date physical on file, please PRINT the “Pre-Participation Athletic Physical Packet” and have it completed by a doctor and turned into the Athletic Office. The “Consent to Random Drug & Alcohol Testing Form” only needs to be completed for High School students.
